چکیده

Sivrioglu AK, et al. BMJ Case Rep 2017. doi:10.1136/bcr-2017-220293 DESCRIPTION A 49-year-old-man presented to orthopaedics clinic with complaint of recently growing thigh pain. He had a history of oral and inhaler forms of steroid usage for asthma and allergy on his background. X-ray examination of the left femur showed medullary lytic lesions localised in distal diaphysis– metaphysis causing cortical destruction (figure 1). Contrast-enhanced MRI revealed medullary bone infarcts in distal diaphysis–metaphysis of the distal femur and proximal tibia bilaterally (figure 2). In this infarct area, there were multiple lesions causing cortical destructions with soft tissue components, and showing prominent enhancement on postcontrast images. The largest one is measured 4×2.5 cm. Multiple smaller lesions which were presenting similiar imaging characteristics without soft tissue components were also seen. Histological evaluation revealed the diagnosis of osteosarcoma. Secondary osteosarcoma is the term referred to osteosarcomas developing on the background of conditions like Paget’s disease, fibrous dysplasia and bone infarct. Osteosarcomas developing secondarily on the ground of bone infarcts are quite rare in this group. Infarcts can be seen in the diaphysis, metaphysis or epiphysis in long bones. Epiphyseal bone infarcts can be recognised and treated at an early stage since they are usually symptomatic. However, diaphyseal and metaphyseal infarcts may not get an early diagnosis since they can be asymptomatic for a longer time. Therefore, neoplasia development on the background of infarcts frequently occur in the medullary cavity of diaphysis. The most common malignancies arising on this ground are pleomorphic undifferentiated sarcoma and osteosarcoma.

Treatment and prognosis for synchronous multifocal osteosarcoma in 42 patients.

Between 1986 and 2002, 42 patients with synchronous multifocal osteosarcoma were treated with two different protocols of neoadjuvant chemotherapy. When feasible, the primary and secondary tumours were excised as a combined procedure. After initial chemotherapy 26 patients were excluded from simultaneous excision of all their secondary bone lesions as their disease was too advanced. In 12 patien...

Synchronous multicentric osteosarcoma in Puerto Rico: a pediatric case report.

Synchronous multicentric osteosarcoma is a very rare condition that might be confused with metastatic osteosarcoma. We report the first pediatric case of synchronous multicentric osteosarcoma reported in the Puerto Rican. The child presented with multiple osteoblastic bone lesions followed by right upper lid ptosis. Orbital CT-Scan showed a lytic lesion involving the sphenoid bone. Management c...

Synchronous multifocal osteosarcoma: results in twelve patients treated with neoadjuvant chemotherapy and simultaneous resection of all involved bones.

BACKGROUND Multifocal osteosarcoma is a rare type of tumor which is always excluded from therapy trials and is therefore rarely investigated for prognosis and treatment. PATIENTS AND METHODS Twelve patients with synchronous multifocal osteosarcoma underwent neoadjuvant chemotherapy and, when feasible, simultaneous resection of involved bones. RESULTS Four patients were unsuitable for resect...

Synchronous Multifocal Osteosarcoma: Case Report and Literature Review

Background. Multifocal osteosarcoma is usually described as the occurrence of the tumour at two or more sites in a patient without pulmonary metastases and may be synchronous or metachronous. Case report. A previously well 21-year old male, who presented with a swollen, painful right knee with no history of trauma, was found to have a high-grade osteosarcoma of the distal tibia and proximal fem...

Bilateral metachronous osteosarcoma of the mandibular body: a case report.

Osteosarcoma of the jaw (JOS) is a rare phenomenon constituting 5% to 13% of all cases of osteosarcoma (OS). JOS has histological features similar to OS, but the biological behavior is different. The chief complaint of patients with long bone OS is pain, whereas patients with JOS usually have painless swelling as the first sign. OS may affect multiple sites. Multifocal or multicentric osteosarc...
